If you would like an in the District Directory, please be sure to fill out the Advertising Order Form sections.
After you’ve completed this form, you’ll need to:
(1) Mail your check, payable to "CML Schools Foundation" to 230 Doherty Drive, Larkspur, CA 94939. Please reference the contributing business on the check.
(2) If you want a space in the District Directory, submit your ad with your check (SEE specifications).

AD ORDER FORM INFORMATION
If you would like a “thank you” space in the District Directory, please also complete the following:

I. Please include my business in the District Directory. I’d like to use:

	LAST YEAR’S AD, as is. No changes, please! I will send a signed copy of last year’s ad with my donation check.
	LAST YEAR’S AD, with changes. I will mark changes on a hard copy and mail the changes with my donation check. Hourly design fee applies.
	NEW AD, emailed in approved format. See formats and specifications. Email to ads@cmlsf.org.
	NEW AD, mailed on CD or Zip disk. I will send a digital file on CD or Zip disk and include a hard copy of my ad with my donation check.
	NEW AD, custom. I would like you to create a new ad for me - I will send a business card / logo / copy / photo / other image (digital or hard copy) with my donation check. Hourly design fee applies. A proof will be sent to you for your approval. Please be sure to supply your email address above.
	NO ad, thanks. I do not want to place an ad, but please do list my business.
	NO ad, no listing. Do not include my business in the Directory at all; I wish to remain anonymous
